Before You Visit - What to Bring

To facilitate your visit to Dr. Cohen's office, please bring the following items. They will help ensure that you visit is seamless and will expedite your care.

• Your insurance card
• Referrals
• A list of prescription or other over-the-counter medications you are currently taking
• Any diagnostic studies or blood work results

Patient Information Form

For your convenience, we ask that you please print out and complete our New Patient Registration Form and Patient Health Questionnaire prior to your first visit. It will save time when you arrive for your appointment and will allow you to patiently fill out the forms in the privacy and comfort of your own home.
